ATTENTION PARENTS

Re: Up to Date Immunization

The Leeds, Grenville and Lanark District Health Unit has mailed a questionnaire to parents of students in grades 9 to 11 who do not have up-to-date immunization information on file with their office.  The Health Unit is required to maintain an up to date database of immunization records for all students.  Schools do not provide the Health Unit with updates on immunization information.  Parents who have received a questionnaire from the Health Unit are reminded to return it to the Health Unit by March 14, 2014.  Should you have any questions regarding your child’s immunization, please contact the Health Unit at 613-345-2566.
